While some states require state-specific forms, there are two common resale certificate forms that can be used for most states:
- MTC Form: Uniform Sales & Use Tax Resale Certificate Multijurisdiction Form
- SSUTA Form: Streamlined Sales and Use Tax Agreement
If you prefer to complete a single form to cover multiple states, follow these instructions:
How to Fill Out the MTC Form

Issued to Seller: Enter the following details:
- Seller Name: “TapStitch, Inc.”
- Address: “8 The Green, Ste B, Dover, DE, 19901”

Buyer Information: Enter your legal name (or company name) and legal address. Ensure the name matches the information you provided in the Legal Info section of your TapStitch Dashboard.

Business Type: Select your registration type:
- Most users will choose “Retailer”, but “Wholesaler” or “Seller” are also valid if applicable.
- If you choose “Other”, specify your business type (e.g., “ecommerce” or “online shop”). Certificates marked “Other” may require additional review by our system.

Business Description: Provide a brief description of your business and the types of tangible goods you sell (e.g., “custom apparel and accessories”).

Seller’s Permit or Business ID Number:
- In the green table, list your seller’s permit or business ID numbers for all applicable states. You don’t need to complete a separate form for each state—just include all states in one form.
- When you upload this form to our system, select all states where you wish to apply the certificate.

Signature and Date:
- Add your handwritten or digital signature, along with the date. This is required for approval.
How to Fill Out the SSUTA Form

Buyer Information: Enter your legal name (or company name) and legal address.
- Seller Information: Enter “TapStitch, Inc.” as the seller’s name and “8 The Green, Ste B, Dover, DE, 19901” as the address.
- Ensure your legal name matches what you entered in the Legal Info section of your TapStitch Dashboard.

Business Type (Section 3):
- Indicate your business type (e.g., retailer, wholesaler). Our certificate system will verify whether your business qualifies for an exemption.

Tax Exemption Reason (Section 4):
- Choose your reason for the tax exemption. This will also be validated by our system.

State-Specific Information (Section 5):
- Select the state(s) where you’re claiming the exemption and provide your ID number and reason code for each state.

Signature and Date:
- Add your handwritten or digital signature along with the date. Unsigned certificates will not be accepted.

Additional Requirements for Certain States
In some states, you must also provide a copy of your seller’s permit along with your resale certificate for verification. This applies to submissions for the following states:
